How can I check on the status of my vital record order? top

To obtain the status of an order you have submitted, use the "Check Order Status" service. Simply enter your confirmation number to view your status.

How can I obtain Vital Record copies? top

There are several options for obtaining copies of vital records. These include:
Order online at http://www.nj.gov/health/vital
Online orders take approximately 2-3 weeks from the date the order is approved plus shipping time. This time is not guaranteed and may be longer or shorter based upon the number of requests received.

In person orders can be completed same day if you come before 3:30pm. For applications received after 3:30 p.m., records will be mailed the next business day.

I would like to cancel my order. top

You may request a cancellation and refund for your order ONLY if the order is still in Received or Deficient status by calling the New Jersey e-Government Services Help Desk. Once your order is approved for processing, it cannot be canceled or refunded.

If your order is canceled, you will still be charged the $5 non-refundable ID Verification Fee, but you will receive a refund of all other fees for your order. You will receive an email confirming your order is canceled and the refund will be applied to your card approximately 5-7 business days from when the order is canceled.

I would like a refund for my order. top

If your order is still in Received or Deficient status, you may request your order be canceled by calling the New Jersey e-Government Services Help Desk. If your order can be canceled you will be issued a refund of all fees excluding the $5 non-refundable ID Verification Fee. The refund will be applied to your card approximately 5-7 business days from when the order is canceled.

Orders that have begun processing or have been completed cannot be canceled or refunded.

Uploading Supporting Documents

I have a hard copy of my document, how can I upload it? top

One of the easiest ways to convert a physical (hard copy) document into an electronic (soft copy) document is to use a scanner. However, if you do not have a scanner available, then taking a picture of the document and emailing or downloading it to your computer will also work.

What kinds of documents will I need for submitting my vital record order? top

To confirm what documents you will need to upload for your vital records order, please use the Document Selector. The document upload page will also provide instructions for the type of documents you will need to upload.

  • Allowed document types include: JPG, JPEG, TIF, TIFF, PDF
  • A maximum of 3 files can be uploaded
  • Files must be less than 4 MB in size

Can I upload a Word document? top

The service does not allow for the upload of Word files. However, 2007 Microsoft Office and later versions allow you save your Word or Excel file as a PDF. Select “Save As” and choose PDF as the file type. You can then upload the PDF version of your Word file. For more details, you can visit the Microsoft Office support site.

What can I do if my file is more than 4 MB? top

To reduce the file size of an image:

  • Adjust the Quality (0% to 100%) of the image when selecting “Save As”. Be sure to review the image to ensure it is readable before you upload it to your order.
  • If you have an image editing program (i.e. Paint) you can crop out unwanted parts of the pictures to reduce the size.

To reduce the file size of scanned documents:

  • Scan the document at a lower resolution (200 dpi is suitable for text documents)
  • Do not choose to scan in full color, opt for greyscale or black and white for text documents

Can I upload BMP/GIF files? top

No, BMP/GIF files are not allowed for upload with this service. However, these files types can be saved as one of the acceptable file types (JPG, JPEG, TIF, TIFF, PDF) and then uploaded to your order. Image editing software such as Paint, can be used to convert the file to a different file type through the "Save As" menu option.

I received an error when trying to upload my file(s). top

Please make sure that your document has the correct file type (JPG, JPEG, TIF, TIFF, PDF) and is within the size limit of 4MB. Also check to make sure that your file name does not contain any special characters other than the period before the file type as other special characters or symbols could prevent the system from being able to upload your document. If you are still unable to upload your document, please try renaming the file with a shorter file name.
Lastly, if you are unable to upload your document after following the above steps, your document/file may be corrupted or have an error. Please make a new copy/scan of the document to upload.

Processing and Shipping

How long does it take to process an order? top

Orders placed through the online Vital Records Service take approximately 2-3 weeks for processing from the date the order is approved plus shipping time. This time is not guaranteed and may be longer or shorter based upon the number of requests received. The shipping time for your order will be dependent on which shipping option you select for your order (USPS First Class or UPS Next Day Air).

If I select Next Day Air for shipping, will I get my vital record the next day? top

No, selection of the mailing option Next Day Air does NOT shorten the processing time. These orders will still take approximately 2-3 weeks to process after approval, but you may see a reduction in the shipping time after the order is completed.

I need my Vital Record sooner than the standard processing time. What should I do? top

If you need to receive your certificate quicker than the standard processing time and live in or near New Jersey, you can visit the Office of Vital Statistics and Registry Office in Trenton between the hours of 8:30am to 4:00pm Monday thru Friday. Please note the office is closed for all state holidays.

We recommend that you call the office at 866-649-8726 in advance to ensure that you bring the proper documentation.

Can I place an order for a Vital Record at my local Vital Records office? top

You can place in person orders for Vital Records at your local registrar’s office only if the event occurred in that municipality. You can visit http://www.state.nj.us/health/vital/order-vital/local-vital-records to find your local office. We recommend contacting the local office before visiting.

Can I place my order by mail? top

Yes. Mailed applications will take approximately 2-3 weeks to process. The application for mail-in requests can be found at: http://www.state.nj.us/health/forms/reg-27a.pdf

You will need to include a check or money order for $25 made out to “State Treasurer” with your application. DO NOT send cash. Additional copies of a record are $2 each.

Technical

What happens if I don't finish my application and close my browser? top

All order information will be lost. You will need to start your order over from the beginning. It is strongly recommended that you complete your order before closing your browser.

What if my PC crashes while I'm working on my order? top

All information will be lost. You will need to start over from the beginning.

How long can I leave my browser open with no activity without losing my order? top

For security purposes, after 20 minutes of inactivity all information will be lost.
